Vestibular anatomy & limits of HINTS

Separate balance concepts from a clinical rule that requires trained examination.

Updated:

Learning objectives

Balance combines several inputs

The inner ear contains structures for hearing and balance. Balance also depends on visual, proprioceptive and central processing. A symptom such as dizziness does not automatically identify a lesion in one inner-ear structure. Begin by distinguishing what a person reports from what an examination observes.

HINTS is not a universal dizziness test

GRACE-3 recommends HINTS for trained clinicians assessing appropriate acute vestibular syndrome presentations with nystagmus. It is not a self-test or a generic screen for every dizzy patient. Central or equivocal findings require further clinical evaluation. A dropdown simulator cannot demonstrate examiner skill, establish the correct clinical setting or exclude stroke.

Practical reasoning station

Teaching case: someone uses an online HINTS animation to conclude that episodic dizziness is harmless. Identify the reasoning errors without making a diagnosis.

Show explanation

The user has not established the appropriate syndrome, trained examination or reliable findings. An animation is not a clinical examination, and a reassuring output cannot exclude serious disease. Discuss the tool's scope rather than labeling the person's condition.

Practice questions

Practice score: 0 / 0

1. Who should apply HINTS clinically in its appropriate setting?

Show explanation

Answer: A clinician trained in its use

Training and the appropriate clinical presentation are central to interpreting HINTS.

2. Can this simulator exclude stroke?

Show explanation

Answer: No

Selections describe a fictional teaching pattern, not an examined patient's risk.
